Pandemic Understanding Form for Families
If your student:
  • has symptoms of COVID (see Symptom Checker linked below)
  • is a close contact of a confirmed POSITIVE COVID case
  • is a close contact of someone awaiting test results for a possible COVID case
  • has tested positive for COVID

SJCA expects all families to screen their student’s health daily.

If your student answers YES to ANY of the questions in the symptom checker linked below, he/she needs to stay home from school and not participate in extracurricular activities until cleared by the SJCA COVID Response Team.

Our Shared Responsibility

As SJCA plans for in-person, daily instruction this school year, we are committed to the health and safety of our school community.  SJCA will continue to consult with and/or seek guidance from the Centers for Disease Control (CDC), the Kenosha County Public Health Department, the Archdiocese of Milwaukee, the Wisconsin Department of Public Instruction (DPI), Kenosha Unified School District (KUSD), and peer institutions.

Vaccinations

Vaccinations help in mitigating the spread of COVID-19 and protecting the health of the school community. SJCA encourages families to consult with their healthcare provider regarding vaccinations.  The Kenosha County Public Health Department provides detailed information on local vaccination clinic locations.
